 It takes a great deal of money to conduct a thorough search and go through all the processes necessary to put a physician on staff: the interview and offer, licensure, signing bonuses, moving expenses, possibly new equipment to initiate a new specialty practice, to name a few of the costs. But consider the revenue a new physician adds above the line and it''s easy to see why demand is so high, and why sooner is better when it comes to filling physician vacancies.Any Warm BodyAny warm body will do. That mindset created the recruiting industry, and that mindset allows it to thrive, but by creating and catering to low expectations, retained recruiting firms have gotten a bad rap -- and deservedly so. Just ask any medical executive who has worked with a retained firm and you''re almost certain to be regaled with horror stories. Data and Key VariablesData used to identify the determinants of having a regular doctor or site and how having a regular doctor or site affects the utilization of prevention were extracted from the Medical Expenditure Panel Survey (MEPS), conducted by the Agency for Healthcare Research and Quality and the National Center for Health Statistics (Agency for Healthcare Research and Quality 1999). The household survey component from 1996, which contained detailed data on demographic characteristics, health conditions, health status, use of medical care services, charges and payments, access to care, satisfaction with care, health insurance, and income and employment, was used. More than twenty thousand individuals representing the U.S. population were surveyed. More details on the information collected by the MEPS 1996 can be found in other studies (Cohen 1997; Vistnes and Monheit 1997). Similar approaches, although limited in number, have been applied to nursing homes. Several clinical trials of QI have not been encouraging (Schnelle et al. 1998; McKenna, Moyers, and Feurberg 1998). Additionally, Zinn et al. (1997) found no association between QI adoption as determined from a survey of nursing home administrators, and two outcome measures: percent of residents with pressure ulcers and percent with contractures. However, these outcome measures were limited in that prevalence (as opposed to incidence) of these conditions may be more influenced by admission patterns than by care actually provided in the nursing home, and the measures were not risk-adjusted. We address this issue in testing the hypothesis: [H.sub.5]: Quality improvement implementation in nursing homes is associated with better risk-adjusted patient outcomes.
