 ResultsAfter assessing the standard of care for each therapy, a cost analysis was performed taking into consideration the number of office visits required for eradication of the wart, physician costs for delivery of the therapy, and medication costs (Figure 1).[FIGURE 1 OMITTED]The costs for the treatment of non-genital warts using cryotherapy with liquid nitrogen, topical squaric acid, carbon dioxide and pulsed-dye lasers, intralesional bleomycin, intralesional interferon alpha, or intralesional Candida immunotherapy ranged from approximately $157 to $1227. Based on our data, carbon dioxide laser therapy was found to be the least expensive ($157). Immunotherapy utilizing Candida antigen was found to be the next-least expensive treatment option for the treatment of non-genital warts ($190), requiring an average of three treatments and a minimal medication expense (Table 4) (12-13). In contrast, interferon alpha was the most expensive modality ($1227), requiring an average of fifteen treatments (Table 5) to attain a complete response. II. THE NATIONAL NURSING HOME ENVIRONMENTThe percentage of elderly Americans is growing.(12) In 1997, 34.1 million elderly accounted for 12.7% of the U.S. population.(13) Roughly 4% of that elderly population elderly received care in 17,176 nursing homes providing approximately 1.8 million beds.(14) Expenditures to care for this population totaled $78.5 billion in 1997,(15) with Medicaid (48%) and private pay sources (31.4%) shouldering most of the burden.(16) The entrance of the Baby Boomers into the 65 and older age categories will double the number of elderly persons to approximately 70 million, or 20% of the population by 2030.(17) This growing elderly population will need adequate long-term care resources to meet its needs,(18) Although nursing homes are not always the preferred method of long-term care,(19) their importance and necessity in our society will only increase as America ages. The function of a usual source of care has been extensively discussed in the health services, public health, and medical literature. Age, gender, race, ethnicity, income, education, insurance status, and health status were found to be common correlates of having a regular source of care (Hayward et al. 1991; Hayward et al. 1988; Merzel 2000; Newacheck, Pearl et al. 1998; Newacheck, Stoddard et al. 1998; Rask et al. 1994; Schur, Albers, and Berk 1995; Shi 1999; Weinick and Drilea 1998; Zuvekas and Weinick 1999). Having a usual source of care was demonstrated to be positively correlated with an individual''s general access to the health care system (Weissman et al. 1991), improvement of patients'' satisfaction with medical care (Hurley, Gage, and Freund 1991), a decrease in the use of the emergency department (Newschaffer et al. 1999), a reduction in the likelihood of hospital admission (Weiss and Blustein 1996), and promotion of proper use of medication (Smith and Kirking 1999).
